Nomination form
Purpose
- To honor the profitable efforts of one Chapter,
individual and/or group who has dedicated
exceptional service in fundraising towards AMTA
goals
- To assist the AMTA with financial needs on a
Chapter level
- To benefit the community
History
- Award established in 1996
- Has been given on an as needed basis since
that time
Qualifications
- Recipient does not have to be a present or past
member of AMTA
- Recipient must have contributed to AMTA and/or
the massage profession in a meaningful manner
- Recipient may receive only one (1) National individual award in any given year. For example: Recipient may receive the Fundraising Project of the Year Award and the Law & Legislation Committee Award; however, recipient may not receive the Fundraising Project of the Year and the Distinguished Service Award
- This award may be given posthumously
- Recipient must contribute to the massage profession
in an outstanding manner
- Recipient shall not have received this award in the
previous year
- Recommended by the Awards Committee
- Approved by the Executive Committee
